Family Information Update 2020

Thank you to our families who have already completed the Family Information Update for the 2020-2021 school year. If you have not done so, please complete the update by Monday, Aug. 3. In order for your student to fully participate in on-campus learning or Mehlville@Home, this update must be completed and submitted.  Log in to Parent Portal to begin.

The Family Information Update gives you the opportunity to review and update your phone numbers, email addresses and other important information. You will be asked to review our annual notifications, media release, medical records, technology and video conferencing policies. 

Parent Instructions for completing the Family Information Update

  1. Log in: Log in to the Parent Portal on a laptop or desktop to begin. If you do not have a log in, you can start the process of getting one by completing a Parent Portal User Request Form and returning it to the school.

  2. Dashboard: After logging in to Parent Portal, you will see a yellow bar in your Dashboard that says "Online Registration must be completed for the 2020-2021 year." Click "View" to begin updating your family's information.

  3. List of forms: The next screen will contain the list of forms to be completed. Click "Edit" next to the first form, "Welcome and Instructions."

  4. Welcome and Instructions: Use the dropdown to select that you are an existing family. Check the e-signature box in the bottom left corner and select "Next."

  5. Household Parents: Verify and update the information in this section. Check the e-signature box in the bottom left corner and select "Next."

  6. Household Address: Verify and update the information in this section. Check the e-signature box in the bottom left corner and select "Next."

  7. Proof of Residency: Answer the required questions in this section. Only families that have had a change of address or lease their home need to provide proof from both List A and List B. If you own your home and have not had an address change, select "Provide documents to the building" in the dropdown. Check the e-signature box in the bottom left corner and select "Next."

  8. Student Information: Verify and update the information in this section. Check the e-signature box in the bottom left corner and select "Next."

  9. Emergency Contact: Verify, update or add information in this section. Check the e-signature box in the bottom left corner and select "Next."

  10. Non-Resident Parent: Verify or add information in this section. Check the e-signature box in the bottom left corner and select "Next."

  11. Legal Documents: This section is to upload any legal documents other than a birth certificate, passport or green card. If this does not apply to your child, select "Not Applicable" from the dropdown. Check the e-signature box in the bottom left corner and select "Next."

  12. Annual Notification: This section must be completed on a laptop or desktop. Click "Annual Notifications." This will launch a new browser tab. After reviewing, return to the original tab. Check the e-signature box in the bottom left corner and select "Next."

  13. Enrollment Survey: Answer all questions in this section. Check the e-signature box in the bottom left corner and select "Next."

  14. Transportation Information: Answer required questions in this section. Check the e-signature box in the bottom left corner and select "Next."

  15. Student Health Information: Answer required questions in this section. If your child received a new physical, it should be uploaded here. This is a requirement for students entering grades K, 4, 7 and 10. If you received an updated immunization record, it should be uploaded in this section as well. Check the e-signature box in the bottom left corner and select "Next."

  16. Student OTC Medications: Answer required questions in this section. Check the e-signature box in the bottom left corner and select "Next."

  17. Student Media Release: Answer required question in this section. Please note that selecting "No" means your child will not be included in the yearbook.

  18. Technology Usage: Click and review all three links and answer the required question in this section. Check the e-signature box in the bottom left corner and select "Next."

Reminder: Obtain your child's immunizations for the 2020-2021 school year

The Missouri Department of Health and Senior Services will not be making exceptions for vaccination requirements for the upcoming school year. Please contact your family medical doctor, urgent care or the public health department to get your student's immunizations prior to the start of the fall semester.
